A late cancellation or no-show is considered a Missed Appointment.
Initial Psychiatric Evaluation – $275*
Follow-Up Visits – $150*
NOTE: Sliding scale may be available depending on circumstance and availability, including student and flexible payment options.
We understand that life can be unpredictable. If you need to cancel or reschedule your appointment, please provide at least 48 hours’ notice before your scheduled time.
When your appointment is booked, that time is reserved exclusively for you. Cancellations made with less than 48 hours’ notice prevent other patients from receiving care.
You will be financially responsible for appointments not canceled or rescheduled within the appropriate timeframe, though consideration may be given for circumstances beyond your control.
Appointments canceled or missed with less than 48 hours’ notice will incur a Missed Appointment Fee.
The Missed Appointment Fee for Initial Psychiatric Evaluations and Follow-Up Visits is $75.
Please let us know at the beginning of a session if time is needed for forms, letters, or notes. We recommend filling out forms beforehand to maximize your session time.
Letters, forms, record reviews, and communication with outside agencies requiring significant attention outside scheduled appointments are subject to additional fees, based on time and service.
This excludes activities deemed clinically essential, such as:
Communication with other clinicians involved in your care
Routine duties (refills, authorization requests, brief follow-up calls)
If additional services are required, referrals can be made to a local psychologist.
Additional fees for phone calls, letters, emails, and reports will be charged based on time and service for established clients. Please feel free to inquire about fees.
Refills are provided during your office visit. Bring your medication list to each appointment.
Please allow at least 72 hours’ notice for refill requests.
Long-term or chronic medication refills require an office visit.
Contact your pharmacy for refill requests; they will fax us the necessary information.
Notify the office during appointments if you are running low on medication to avoid shortages.
For mail-in prescriptions, you are responsible for sending the completed forms.
Under unforeseen circumstances, contact the office during business hours for non-controlled medication refills.
Note:
Controlled prescriptions lost or stolen may require a police report and will only be refilled if clinically appropriate.
Refills do not guarantee insurance coverage — check with your pharmacy regarding medication coverage.
Is your insurance not listed or considered Out of Network?
No problem — we can still help!
If your insurance plan is not one we’re contracted with, you will be considered Out of Network. Many PPO and private insurance plans include Out-of-Network Provider benefits, allowing you to receive reimbursement for care.
